His records be corrected to reflect the following awards; 

 

 1. The Air Force Commendation Medal (AFCM) 

 

 2. The Air Force Good Conduct Medal (AFGCM) (Administratively Corrected) 

 

 3. The Air Force Longevity Service Award (AFLSA) (Administratively Corrected)

APPLICANT CONTENDS THAT: 

 

The awards are not reflected on his DD Form 214, Armed Forces of 
the United States Report of Transfer or Discharge. 

 

In support of his request, the applicant submits copies of his DD 
Form 214 and a signed AFCM certificate.

STATEMENT OF FACTS: 

 

On 23 Oct 70, the applicant was honorably discharged from Active 
Duty. He served 4 years, 1 month and 26 days on active duty.

AIR FORCE EVALUATION: 

 

HQ AFPC/DPSIDR recommends denial of the applicant’s request for 
the AFCM. DPSIDR notes there is strong evidence to support the 
applicant was awarded the AFCM; however, they are unable to 
locate the official source document to substantiate receipt of 
the award.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

He earned the AFCM and wore it proudly. The AFCM was the most 
meaningful accomplishment of his Air Force career and was awarded 
for his meritorious service while assigned to a LEAD TEAM of a 
weapons loading crew. He did not keep track of his special order 
for the AFCM. 

 

He submitted copies of additional documents, including the AFCM 
citation with official seal, as proof of his receipt of the 
award.

Exhibit D.

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by existing 
law or regulations. 

 

2. The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice concerning the 
Air Force Commendation Medal (AFCM). In this respect, we note 
the AFCM certificate and citation, provided by the applicant, 
indicates the decoration was awarded to him on 1 Jul 1969 for 
meritorious service. Although the applicant has not provided a 
copy of the order awarding the AFCM, we find the documentation 
the applicant has provided sufficient to resolve any doubt in his 
favor. Therefore, we recommend his record be corrected as 
indicated below

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air Force 
relating to APPLICANT be corrected to show that the DD Form 214, Armed Forces of the United States Report of Transfer or 
Discharge, issued in conjunction with his 23 Oct 70 separation, 
be amended in Item 24, Decorations, Medals, Badges, 
Commendations, Citations and Campaign Ribbons Awarded or 
Authorized, to include the Air Force Commendation Medal (AFCM).
